Project CLEAR is a post-discharge decolonization protocol designed for patients infected or colonized with a multidrug-resistant organism (MDRO). The Changing Lives by Eradicating Antibiotic Resistance (CLEAR) study, published in NEJM in 2019*, showed that patients colonized with methicillin-resistant Staphylococcus aureus (MRSA) - who used a serial decolonization protocol for six months after hospital discharge - had the following results:
• 30% reduction in MRSA infection in the one-year post-discharge period
• 17% reduction in all cause infections
Due to these impressive results, Hoag Orthopedic Institute (and other Orange County hospitals who chose to participate) were provided with CLEAR kits to offer patients on MDRO contact isolation upon discharge. These kits include 4% chlorhexidine (CHG) liquid with mesh sponges for showering and nasal iodophor swabs, to be used on a daily basis for 5 days every other week for a 6-month period.
In keeping with our mission to provide safe patient care, in 2019, HOI began offering CLEAR kits to MDRO colonized patients upon discharge.
