PERFORMANCE IMPROVEMENT
HOI is committed to an active Performance Improvement (PI) program for maintaining a constant state of awareness of the systems and processes that affect patient care. This awareness is critical to identifying and preventing defects, and pinpointing opportunities for improvement. PI projects incorporate LEAN techniques, pioneered by the Toyota Motor Corporation in the manufacturing sector and increasingly accepted in the healthcare industry as the gold standard for process improvement. Continuum of Care between hospital and SNF/Rehab centers and Home Health Agencies [facilities] is not consistent. • Improve communication and follow-through of care/instructions between hospital and facilities. • While the readmit rate is low, improvement is needed to prevent and reduce risks of post op complications and risk of readmissions and costs. • Readmissions within 30 days of DC will be a penalty in the future.
Create a unified analgesic pathway/practice for elective primary total knee patients for HOI.
To prevent falls with a goal of zero falls for HOI.
The city of Irvine has a large Persian community, many of whom are primarily Farsi speaking. Healthcare workers are often not familiar with cultural differences or able to communicate without an interpreter.
To identify and reduce repetitive process/flashing of specialty surgical instruments. Once instruments are identified, take action by either acquiring or removing them from the trays for use.
Streamline the care of fractures patients presenting to the ED to ensure timely surgical interventions as well as medical management of chronic conditions.
Some patients perceived that staff did not explain side effects of new medications that were being administered. Goal to increase patient medication education scores 5% within 2 quarters.
