READMISSION RATES
For all-cause readmissions in 2018, HOI had the sixth lowest rate in the entire nation based on data from the Health Services Advisory Group (HSAG) Inc. Following elective surgical procedures, the readmission rate shown at right includes unplanned readmissions within 30 days of hospital discharge – related as well as unrelated to the elective surgical procedure.
READMISSION RATES FOR PATIENTS ON HIGH-RISK MEDICATIONS
High-Risk Medications (HRM) are those defined by an increased risk of adverse effects. HRMs most commonly impacting care at HOI are opioids, anticoagulants and diabetic agents. Patient education is key to preventing the occurrence of adverse events related to HRM. HOI is committed to eliminating those occurrences. The adjacent HRM readmission rate graph illustrates HOI’s vigilance in preventing adverse drug events from prescribed HRM.
